observe what they might encounter if they are invited to a Human Factors Debriefing (HFD).
The eLearning details the steps of the safety science-aligned Systemic Child Fatality Review
Process, which emphasizes the importance of learners’ participation in case reviews. It also
examines how the process informs systemic improvement over individual blame and how
it contributes to organizational learning, continuous improvement, staff wellness and
improved family outcomes.
